I/We understand and agree that the Hospital Authority (“HA”), its IT Innovation Office and partners have the sole and absolute discretion to: (a) use, amend or share among themselves the information provided; and (b) amend and publicise such information indicated on the form that would be published online. I/We shall not claim against the HA or its partners for damages arising from such use, sharing and publication. I/We shall indemnify and keep the HA fully and effectively indemnified against any allegation of or claim for infringement of intellectual property rights (including without limitation copyrights / patent issues) of any person or party arising from or in any way related to the provision of any information by me/us or any of our employees or agents.I/We understand that neither the invitation nor the submission of solution proposal or related information constitutes a request for quotation or a call for tender. The HA has the sole and absolute discretion to decide whether to proceed with a quotation or a tender, and how to proceed. Hospital Authority owes no obligation to a company/organisation that submitted a solution proposal, and shall not be responsible for any cost incurred by the company/organisation for the preparation of the proposal.

Purpose of Collection
The personal data collected by means of this form will be used for the following purposes:
Processing I&T solutions of your organisation/company;
Publishing on Internet and the HA internal platforms as market information sharing;
Recording and preparing statistics; and
Facilitating communication among the HA’s departments/partners and your organisation/company.
Transfer of Data
The personal data provided by means of this form may be disclosed to the HA’s partners.

Access to Personal Data
Subject to exemptions under the Personal Data (Privacy) Ordinance, you have the right to request access to or correction of personal data provided. Such request should be sent to the HA’s IT Innovation Office by email to innovation@ha.org.hk.
